Insurance
Payment is expected at the time of service. This includes co-pays and
co-insurance if you are covered by insurance.
We participate with many different insurance plans, both PPO's and HMO's.
It is your responsibility to know what your plan covers and excludes. If
we are participating with your plan, we will be happy to file a claim for you.
If for some reason your claims are denied by your plan, it is your
responsibility to remit the full payment to our office within 15 days of the
denial. If you are unable to make this payment, please contact the office
manager to make reasonable payment arrangements. Our fees are competitive
and reasonable and we expect that payment will be made in a timely manner so
that we can continue to serve you as a patient.
